This antibiotic is included on the Formulary restricted to hospital-based use on the advice of a microbiologist or infectious disease physician. Prolonged use (>2 weeks) should usually be avoided. There may be exceptional circumstances when a GP will be asked to prescribe. There is no shared care protocol, but discussions between the GP and consultant can be supported by the individual patient care protocol on Staffnet. The weekly blood monitoring will continue to be carried out by the hospital, unless the GP has agreed to take responsibility. There is a mechanism for GP practices to apply for contingency funding as the cost of 7 days’ treatment is £623.
